What is Follow-Up Consultation?
The Follow Up Consultation is designed to monitor your progress toward your nutrition and dietary goal. During this consultation, it is important for me to obtain your feedback about how you are managing the changes in your lifestyle, diet, and nutritional health. I need to know about your likes and dislikes, what is easy to change and what is challenging. From this information, I may reassess or make changes to your plan, and if necessary, provide you with extra information and advice. The follow-up consultation gives me the opportunity to continue to motivate and help you to achieve your short-term and long-term goals because managing lifestyle, dietary, and nutritional health changes usually involve ongoing care and attention.
To prepare for your visit, it can be helpful for you to write down everything you ate in the last week prior to the appointment (or better yet, a food diary of a week worth of meals and snacks). With that guidance, I can begin to discuss your dietary preferences and cooking habits, and identify trouble spots, like skipping breakfast or late-night snacking.
Who needs the Follow-Up Consultation
The Follow-Up Consultation is a crucial step in ensuring your continued success in reaching your nutrition and dietary goals. Specifically tailored for clients who have already undergone the initial consultation, this follow-up session serves as a dedicated opportunity to monitor and analyse your progress.
Your follow up/Review consultation will cover:
- Evaluating progress regularly and adjusting the dietary plan as necessary.
- Introducing new food options to maintain variety and interest in dietary intake.
- Providing ongoing education on optimal nutritional choices for long-term benefits.
- Determining the frequency of review consultations on an individualised basis.
- Recommending ongoing appointments to be scheduled between 2 to 3 weeks apart.
